Director of Care Management Port Jefferson, New York As Director of Care Management, you will draw upon your four-to-five years of experience as an RN in a clinical hospital environment in the oversight of seven case managers and nine social workers in day-to-day operations. Responsibilities will include scheduling tasks, planning training, budgeting, and keeping abreast of current regulatory requirements, including the Joint Commission, Medicare, Medicaid, and IPRO. You will function as the “go-to” person for physicians and team members and, essentially, represent not only the department, but also the hospital in various committees and operational initiatives. To qualify, you should hold a Bachelor’s degree in Healthcare Management and current New York State licensure and registration as a Registered Nurse. Four to five years of experience as an RN in a clinical setting is sought, with two years in Care Management, including discharge-planning processes. In addition, you should bring strong organizational, interpersonal, and communication skills to the table. Solid computer skills with experience in EPIC and MIDAS programs are preferred.
