Care Coordinator Description: A Home Care Supervisor assumes responsibility for assessments, care coordination, advocacy, and related case management activities for an assigned group of clients. Inherent in the position is the assessment of patient needs in conjunction with the client and/or family, and ancillary health care providers. Job Duties: 1. Assists in assuring that an adequate supply of qualified field staff is available and appropriately assigned on the cases by maintaining a current availability list; scheduling appropriate personal care workers on assignments; and properly documenting assignments and/or communications in the computer. 2. Responsible for scheduling all personal care workers staff to home care cases. 3. Establishes appropriate helpful relationships with field personnel and ensures full time staff are maintained at their appropriate productivity levels through appropriate notification and coordination. 4. Responsible for reporting any staff member who is difficult to assign, constantly refusing assignments and/or not showing up as assigned, etc. 5. Maintains good communication and/or phone coordination among the field staff, patients, referral sources, and co-workers in the office. 6. Answers telephone calls as needed. Attempts to respond to caller needs, or passes to appropriate person for follow-up. Takes messages for those calls that can be returned at a later time. 7. Provides back-up assistance for the payroll process; including accuracy of pay and bill rates.
