The Release to Rent Veterans Program is currently seeking an additional case manager. Case Manager CJM--R2R Veterans Program Coordinator SUMMARY The R2R Veteran Case Manager works with the R2R Veteran team, (coordinator and other case manager), using a multidisciplinary approach, to provide a supportive housing program for up to 30 male Veterans who have a history of homelessness and have a criminal background. The R2RV case manager position is a grant funded position contingent on the continued funding from the Veterans Administration. PRIMARY RESPONSIBILITIES o Planning and carrying out the move and set-up of furnishings of program apartments in preparation for the Veteran to enter program o Assisting in orienting the Veterans to their apartments and surrounding areas o Helping clients to set goals, visiting them at home, keeping case notes, evaluating clients' progress and referring them to social service resources in the St. Louis area o Mentoring the men in the program by example o Observing drug screenings with the male Veterans in program o Facilitating a weekly support group (rotation with other staff) o Having daily contact with clients (by phone on weekends) o Being on call 24/7 for clients PRIMARY QUALIFICATIONS o Is Veteran of the United States Military or Reserves (preferred) o Brings qualities that would balance the other staff of the R2R Veteran team o Has education or experience in social work, substance abuse, or related social service field, preferably as a case manager o Carries out responsibilities without constant supervision and has proven by actions to be trustworthy and reliable o Has shown ability to interact in a positive manner with a diverse population of clients and co-workers o Is willing to work in a Housing First/Scattered Housing/Harm Reduction model o Is able to provide documentation from a physician which indicates the ability to move furniture and other household items on a regular basis o Is willing and able to move furniture and prepare apartments for clients to move in o Must obtain a TB test annually o Is able to work a flexible schedule, which may include early mornings, evenings, and weekends. This includes being "on-call" 24/7 o Must possess valid driver's license, own vehicle, have up to date auto insurance, and be willing to transport clients in personal vehicle. Preferably Class E License or ability to obtain (to drive a box truck/small moving truck) o Lives within the St. Louis metropolitan area (preferred) because this position requires a quick response to client needs on a 24/7 basis o Understands the mission of CJM o Understands the implications of working for a non-for-profit, faith-based service organization o Demonstrates basic computer skills including Word, Excel, Outlook, and internet o Completes other duties as assigned The R2R Veteran Case Manager will carry out the following responsibilities in a caring, accurate, professional and timely manner: Communications- All correspondence will be answered in a timely and accurate manner. Case manager needs to be available by telephone to clients and supervisor 24-hours a day. Case manager will work with each client to complete all required forms and agreements, to be sure each client has copies of all agreements and needed information, and to file CJM copies in a professional manner. Case Manager will attend case staffing with the Program Coordinator and will ensure that any problems, situations or transitions are discussed Case Manager will pass information, regarding events and job leads, to the clients in a timely manner Supportive Housing All clients will be oriented to the assigned housing unit as well as to nearby resources. Staff member is responsible for ensuring the housing is ready for move-in when scheduled and that the client has needed items. Staff member may be expected to meet with the utility company at client's house. Staff member will work with the client and landlord to resolve any issues that may arise. Client Needs Every purchase of needed items (e.g. bus tickets/passes, food vouchers) will be done within the budget and with proper handling of receipts and recordkeeping through the Program Coordinator. Staff will periodically check that clients are gradually learning how to meet their own needs. Staff will give special attention to clients nearing the program conclusion to help the client plan for needed transitions. No client will be required or urged to follow any one specific path to re-entry success. Personal Contacts Case manager will accompany a client to new settings. Staff will be expected to regularly make home visits at a time when the client is home and is also required to do home visits when the client is not home. Recordkeeping The Case Manager will keep professional, current case notes for each client and assist in keeping the client file information up to date with the current contact information. Staff members are responsible for keeping track of their own mileage related to client contacts. Case Manager is responsible for verifying that the client is attending meetings, medical/mental health appointments, and a treatment plan is in place. WORKING CONDITIONS Most duties will be performed out of the office including at the client apartments and in the community working one-on-one with the clients who have various criminal backgrounds. The position will require some weekend and/or evening phone calls and other work. ACCOUNTABILITY R2R Veteran Case Manager is accountable to the R2R Veterans Program Coordinator, a program under the leadership of the Director of Criminal Justice Ministry, who reports to the CJM Board.
