Seeking an Experienced Program Specialist II, Case Manager II and Clinical Manager

Program Specialist II, Case Manager II and Clinical Manager Chicago, Illinois
As our Program Specialist II, you will conduct screening and risk-assessment interventions, per program guidelines, and act as primary liaison for members and staff. You will also assist with non-medical aspects of the members’ care, including referrals to community resources. To qualify, you must hold a Master’s degree in Social Work or a related field and have 4 years’ experience counseling patients with disabilities or chronic conditions. LCSW is preferred. Some travel may be involved.
As our Case Manager II, you will perform collaborative duties to assess, plan and coordinate a continuum of care for select members with complex medical or behavioral health conditions, or those who are at high risk for same. To qualify, you should be a registered nurse with 2 years’ clinical nursing or case management experience.
As our Clinical Manager, you will champion case-management and care-coordination services and activities for members with co-occurring behavioral health and medical conditions, in addition to overseeing our team of licensed medical- and behavioral-health clinicians. To qualify, you should be a registered nurse with 3 to 5 years’ case-management/utilization experience, preferably, within a supervisory role.
