SUMMARY Primary function is to administer skilled nursing care, under the supervision of Registered Nurse for clients of all ages in their place of residence; coordinate care with the interdisciplinary team, patient/family and referring agency. II. EXPERIENCE AND TRAINING GUIDELINES Education: Graduate of an accredited college/university with a degree in vocational nursing. Experience: Requires one year experience as a Licensed Vocational Nurse in a clinical care setting. Licensure: Current Texas State license as Licensed Vocational Nurse. Current Driver License Proof of current CPR, negative TB screen, and Hepatitis profile. III. WORKING CONDITIONS Environment and Working Conditions: • Work in the patient's home in various conditions, possible exposure to blood and bodily fluids and infectious diseases. • Maintaining good body mechanics and sage lifting practices. • Following agency schedule for client visits, client care conferences, in-services. • Instructing the client and others in the role of the home health aide. • Reporting use of medications by clients that are not on the current medication profile (i.e. new meds, questioned or unfamiliar medications, etc.). Physical and Mental Effort: • Prolonged standing and walking required with ability to lift up to 50 pounds and move patients. • Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition; meet patient/family individualized psychosocial needs. • Requires hand-eye coordination and manual dexterity. Transportation: Must have reliable transportation. Valid and current auto liability insurance. IV. QUALIFICATIONS Ability to: • Maintain good body mechanics and sage lifting practices. • Follow agency schedule for client visits, client care conferences, in-services. • Instruct the client and others in the role of the home health aide. • Report use of medications by clients that are not on the current medication profile (i.e. new meds, questioned or unfamiliar medications, etc.). • Holding in confidence all personal information relayed by the client or others, unless such information would be beneficial to the client if reported to the RN or Client Care Coordinator. ESSENTIAL FUNCTIONS STATEMENTS Essential responsibilities and duties may include, but are not limited to the following: Essential Functions: • Collaborate with RN to assess home health patients to identify the physical, psychosocial and environmental needs, as evidenced by documentation, clinical record, case conference, team report, and evaluations. • Participate in implementation and development of the plan of care to ensure quality and continuity of care and proper discharge planning. • Assure continuity of quality patient care delivered. • Monitor assigned cases to ensure compliance with requirements of third party payer. • Demonstrate commitment, professional growth and competency. • Promote Agency philosophy and administrative policies to ensure quality of care. • Must practice safely within the job description • Must attend orientation • Must attend staff development programs • Communicate with Client Care Coordinator and RN every day client visits are scheduled to note schedule changes and report problems. • Complete documentation and turn in visit records, daily visit report and missed report no later than by 5:00 pm on Mondays. • Activities done (per aide care plan) or refusals by client, vital signs, supplies used and RN supervisor notified of any problems. • Observe the client's mental, emotional, and physical status and reporting changes to the registered nurse. • Document missed or refused visits and notifying the RN supervisor of any changes from the written visit schedule and if a visit should have been made. • Participate in Client Care Conferences on a regular basis. • Prepares clinical and progress notes. • Assist the physician and Registered Nurse in performing specialized procedures. • Prepares equipment and materials for treatments observing aspetic techniques as required. • Assists the patient the patient in learning appropriate self-care techniques.
