The Family Nurse Practitioner will work with the KBR Reynolds reverse co-location grant to provide routine care, physical examinations, health screenings, metabolic health indicator tracking, health management, collaborating with the treating psychiatrist and primary care providers in the community, and provide patient education to those enrolled and who are participating in the grant study. Work closely with identified persons on the grant team to provide routine primary care services to identified high-risk individuals for prioritization for primary health care services. Provide initial evaluation and medication management of general medical issues of those individuals enrolled in the grant and referral to those to outside providers or specialists for those in need of a higher level of care. Provide baseline chart reviews to determine medical needs of patients as well as outreach to primary care providers, health and safety partners and internal mental health staff for communication and care coordination and facilitate medical home linkage. Assist with the documentation and monitoring of specific outcome measures as indicated in the grant. Meet on an as scheduled basis with a designated primary care provider in the community for supervision of medical cases.
There are no supervisory responsibilities.
MINIMUM QUALIFICATIONS:
Nurse practitioner must have a nursing degree from a nationally accredited institution of higher learning
Masters degree in nursing with certification from NC nursing board
Fully certified as a nurse practitioner and has a current unrestricted license to practice as a FNP in North Carolina and is in good standing with the medical and nursing boards.
Holds a current, unrestricted DEA license and is in good standing with federal agency
Has a current NPI number
Has individual liability insurance coverage for practice
Has an identified primary care physician who will give adequate supervision
Must be able to work on a part-time or modified full-time basis depending on the amount of individuals enrolled in the KBR grant
