Position Description: Responsible to provide quality professional geriatric care management services to all clients and their responsible parties adhering to the NAPGCM Pledge of Ethics, Standard of Practice and The Care Management Team mission statement. RNs and Social Workers only please.

Geriatric care management is defined as professional consulting which assists clients and their responsible parties with decisions related to aging, available services and long term care planning. Our objective is to assist our clients and their responsible parties in managing and navigating the issues they will face as they age.

The care manager will assess each client and family situation by evaluating the client’s level of functioning, their health, their emotional state, their finances and their legal needs in an effort to enhance their overall quality of life.

* Must be a licensed RN or Social Worker. Experience in medical and long term care settings preferred.

• Ability to provide non-directive guidance and facilitate constructive relationships.
• Demonstrates Flexibility and Accessibility – Office hours are Monday through Friday 8:30am to 5:00pm however a representative of the company is available 24 hours a day, 7 days a week to assist our clients.
• Ability to ensure inappropriate placements, duplication of services, and unnecessary hospitalizations are avoided.
• Quality control – care management services adhere to NAPGCM’S Standards of Practice and Pledge of Ethics.
• Manage time efficiently.
• Ability to provide coordinated communication between family members, doctors and other professionals, and service providers.

Key Responsibilities:

• Responsible for adhering to company mission, philosophies and all policies and procedures.
• Responsible for professional behavior on behalf of clients and in all community venues.
• Act as an advocate for clients, recognizing the client's values rather than own.
• Conduct care plan assessments to identify problems, eligibility for assistance, and need for services.
• Develop, monitor and manage the care plan.
• Screen, arrange, and monitor all services to optimize benefit to the client.
• Review financial, legal, or medical issues and offer referrals to specialists to avoid future problems and conserve assets.
• Identifying resources to address clients’ health, daily living, and safety needs.
• Assist with determining the level of care needed and moving a client to an appropriate facility or arranging appropriate in-home care.
• Accompanying clients to provider and/or physician appointments.
• Monitoring clients’ health status and well-being.
• Provide crisis intervention.
• Act as a liaison to responsible parties and related professionals, whether local or distant, to keep them informed of the client's situation.
• Provide counseling and support to client and their families.
• Educating the client and/or family on aging issues, health, and life choices.
• Communicate effectively and timely with all involved participants.
• Complete documentation requirements accurately and timely.
• Participate in staff and training meetings, community, and professional educational programs to continue to provide accurate information and enhance professional knowledge.
• Provide consumer education.
• Promote company to all referral sources, vendors, etc.
• Participate in marketing efforts with the goal of educating the public, cultivating clients, and securing new clients.
• Demonstrates competency in core care management areas outlined by the company.
• Complete additional tasks as requested by supervisor.
• Expected hours billed

Qualifications:

* Must be a licensed RN or Social Worker. Experience in medical and long term care settings preferred.

• Bachelor's degree or equivalent education required, Master’s degree preferred.
• Experience in local human service and medical field and resources regularly utilized by older adults (i.e. Medicare, Medicaid, etc.).
• Excellent communication, organizational and time management skills.
• Personal integrity and ethical standards.
• Must have reliable transportation and vehicle insurance equivalent to $300,000/$500,000.
• Computer proficiency in Microsoft Word, Excel, and Outlook.
• Physical Abilities: Must be able to sit for long periods, stoop, lift, drive and bend effectively so as to be able to perform the above listed Job Duties & Responsibilities.
• Recent Criminal Background Check, DMV Check and Medical Exam.

This position is an office-based position and requires flexibility to work unconventional hours and local travel to accommodate client appointments.
