NC Licensed Therapist

Innergy LLC is seeking a Licensed Therapist for its Charlotte office. Supervisor for Intensive In-Home team.This position requires Master’s level Licensed professional (LPA, LPC, LCSW) with two-year experience working with children and adolescent who SED.

Qualified Professional will provide quality community support services that will include linking/coordinating/monitoring needed/identified services, providing 1:1 skill building activities to clients and families, and advocating for clients/families. Qualified Professional must be able to communicate clearly and effectively with child/family treatment team, school system, medical/psychiatric professionals, court system, and law enforcement officials. Qualified Professional will be on call 24/7/365/ to clients and families. This position will be responsible for developing, coordinating, and monitoring a comprehensive person-centered plan for each client that meets all applicable guidelines for the population served. In addition, they shall plan and insure facilitation of comprehensive planning team meetings, revising the plan as needed and securing all approvals and signatures for the plan. Plans may not be developed or altered without the input and the specific approval of the client/guardian.

Duties/Responsibilities (other duties may be assigned):
• Update individual service plans with clients and families, as client needs change.
• Incorporate client choice in developing individualized person-centered plan.
• Link clients to the most appropriate and cost effective services through the implementation of client’s person-centered plan.
• Insure that all required assessments (Target POP, NCTOPPS, etc) are performed as required and routine documentation is recorded appropriately and within specified timeframes.
• Insure at least bi-weekly face-to-face with the client and monitor the efficacy of all services the client receives, increasing contacts as client acuity or program procedure may require.
• Hold regularly scheduled monthly team meetings to coordinate the person-centered plan, assuring that all relevant agencies and community members are present.
• Note all changes in diagnosis or medication in the client chart in a timely manner.

Primary duties include coordinating and providing therapy as a member of our Intensive In-Home teams
