Job Summary:
The GAFC Caregiver monitors placement status in the home and provides assistance with participantsí daily living needs as directed by a plan of care ensuring that personal care services are provided on a daily basis.

Duties and Responsibilities:
1. Ensure that the facility and the unit are properly maintained so that is consistently in compliance with all eligible criteria.
2. Ensure that the following are provided as needed:
a. Laundry services for bedding, towels, and washable personal clothing.
b. Supervise and assist with activities of daily living.
c. Cleaning and general housekeeping in participantís area.
d. Assist with shopping
e. Periodic evaluation of participantís clothing to ensure adequate apparel, especially during the colder months.
3. Provide ongoing supervision of the participantís health related activities, such as
a. Reminding participant to take prescribed medications if necessary.
b. Comply with health care instructions explained by the plan of care.
c. Notify HAP RN promptly regarding medical concerns, such as medical emergency if participant becomes ill, any medical changes reported by client or noted by caregiver.
4. Notify HAP staff as soon as possible upon the occurrence of any of the following crises:
a. Fire, accident, injury, or evidence of serious communicable disease contracted by staff/participant.
5. Notify staff promptly of any participant needs that are unmet, such as lack of food, clothing, financial concerns, etc.
6. Maintain schedule and provide advance notice when requesting time off.
7. Provide accurate documentation of services provided.
8. Perform those duties assigned through the service plan and HAP staff.
9. Maintain client confidentially.
10. Participate in on-going CQI activities.


Education and Experience:
Must have successfully completed a Certified Nurse Assistant (C.N.A) and or Home Health Aide (HHA) training program and possess a certificate of course completion and/or a valid license. Equivalent training or experience could be substituted for certification/licensure.
