We are seeking an experienced RN Case Manager to join our patient-minded team. This is an ideal opportunity for a dedicated, hard-working and motivated individual to interact directly with our patients and make a significant impact within our own organization.
Position qualifications include: Registered RN with current California RN license; At least one (1) year of clinical experience, preferably in home health or acute care environment; Current BLS Certification; Previous experience with the OASIS and Medicare Process; Safe driving record & ability to navigate; Flexibility to meet the changing needs of our patients (i.e., distance, scheduling); Demonstrated professional attitude Ability to work independently, as well as in a team environment; Good communication & organizational skills; Computer literacy, experience using an Electronic Medical Record (EMR) preferred.
CVH Home Health Services uses the case management model, with an RN in charge of monitoring the overall caseload. We understand the importance of keeping the physician updated and fully informed. Weekly case conferences are conducted to ensure coordination of care. The patient (or family) is fully involved with developing the plan of care that the primary care physician will ultimately approve. Our Case Managers partner closely with staff clinicians to coordinate services and maintain patient’s plan of care.
